Who is responsible

JM Injury & Rehab Clinic is the data controller for the information you provide. Any questions about this notice, or any request relating to your data, can be sent to jm.injuryclinic@gmail.com.

What is collected

• • Your name, email address and phone number.
• • The visit location and, for home visits, the address or postcode.
• • The service booked and the appointment date and time.
• • Details of your injury and any medical notes you choose to share. This is health information, which is treated as a special category of personal data.
• • If you leave post-appointment feedback, your rating and any comments you write.
• • If you contact the clinic about club or squad work, the details you send.

Why it is collected

To arrange and deliver your appointment, to keep an accurate clinical record of the treatment provided, to confirm and update your booking by email, and to send aftercare guidance and an optional feedback request after your visit. Health information is processed on the basis of your explicit consent, given through the consent box on the booking form, and for the provision of healthcare treatment.

Who it is shared with

Your information is not sold and is not shared for marketing. It is handled by the service providers the clinic uses to run its bookings: the hosted booking database, the email service that sends your confirmations, and the clinic's calendar, which holds the appointment time, service and visit location. Information may also be shared where the clinic is required to by law, or where there is a serious risk to someone's safety.

How long it is kept

Clinical records are retained for as long as professional practice and insurance requirements expect them to be kept, which for adult clients is normally several years after your last appointment. Records for clients treated under the age of 18 are kept longer. Enquiries that don't lead to a booking are deleted once they are no longer needed. If you'd like the exact retention period that applies to your record, just ask.

Your rights

You can ask for a copy of the information held about you, ask for inaccurate details to be corrected, withdraw your consent, or ask for your record to be deleted where there's no legal or professional reason to keep it. Email the clinic and your request will be answered within one month. If you're unhappy with the response, you can complain to the Information Commissioner's Office at ico.org.uk.

Reviews on this website

Feedback you leave after an appointment is private by default. A review is only shown on this website if the clinic marks it for display, and only ever as a rating, your comment and a first name — never your contact details or injury information. Ask at any time and a published review will be removed.

Cookies

This site doesn't use advertising or tracking cookies. Only the storage needed to keep the booking and clinic login working is used.
